Welcome to Florence Chapel Middle School! We are excited that your student will be joining us here. You will find a link below that will allow you to choose a time for a registration appointment. We do this to streamline the process and ensure that we can give you the support and assistance that is sometimes required while registering a new student.
To register a new student we kindly ask that you bring the following to your registration appointment:
-Birth Certificate
-Current immunization records
-Proof of residence (utility bill, mortgage statement, lease agreement etc...)
-Last report card or transfer grades from previous school
-If applicable: proof of custody, ESOL, IEP, 504, and/or GT paperwork
