Physicals must be dated April 1, 2025 or later to be valid for the 2025-2026 school year.

To be eligible for athletic tryouts, follow these steps: 

• Print a copy of the Athletic Physical Form Pack for 2025/2026 by clicking here, or pick one up from the ACM Front Office.

• On 4/1/2025 ReGenesis Health will be at Abner Creek Middle School. All parents need to complete this BEFORE your student comes in for a physical.

• Go to Big Teams and create an account, the athlete and parent have to have separate accounts that will link together.

• Upload athletic physical forms and complete documents.

• All documents are required BEFORE any workouts or practices.
