Physicals must be dated April 1, 2024 or later to be valid for the 2024-2025 school year.

To be eligible for athletic tryouts, follow these steps: 

• Print a copy of the Athletic Physical Form by clicking here, or pick one up from the ACM Front Office.

• Go to an urgent care, minute clinic, or your personal physician and have the physical form completed - it must be signed and dated by a physician 4/1/2024 or later to be valid from 6/1/24 - 5/31/25.

• Go to Big Teams and create an account, the athlete and parent have to have separate accounts that will link together.

• Upload athletic physical forms and complete documents.

• All documents are required BEFORE any workouts or practices.
